Q ParentConnection is a student information portal for parents/guardians. As part of the re-enrollment process, please update demographic information for each student.
- Q ParentConnection re-enrollment opens July 21, 2026.
- Instructions for the process are at the bottom of this page.
- Login with your Parent ID or email that is already in the Q system.
- Your Parent ID and/or email can be reset from the ParentConnect login screen.

MCMS Registration Information:
We look forward to seeing you soon. See below for more information about in-person registration and picture day at MCMS:
- Picture day is coming soon! This year, picture day will take place July 28th - 31st, during in-person registration (see schedule below.) Parents/ Guardians may register for students during the in-person registration.
- Please note: Student ID Photos will be taken at in-person registration 7/28-7/31. Student IDs will ONLY be created for students who attend registration.
- Make up photos can be taken at a later date.

Mandatory Forms to Complete, PRINT & BRING to Registration
- Re-Enrollment Confirmation: Re-enrollment review confirmation page from Q Parent Connection (See instructions below).
- Health Information and Medical History Mandatory.
- Medication Authorization form (if applicable).
NOTE: These forms are available on Q Parent Connection and are required to be turned in on the day of registration in order to receive the student's schedule, assignment book and locker assignment.
Please print at home and bring to registration. If you do not have a printer we will have a print station available. Please take a screen shot or email yourself the electronic document and bring to Registration for easy retrieval in the lab.

- TDAP Proof of Vaccination (entering 7th graders only)
- Spirit Wear - Get you spirit wear swag between July 22 and August 9
- Volunteer Form: Sign up to be notified about volunteer opportunities
- Voluntary Carpool Participation Form: Interested in carpooling? Let PFA know and we will help match you with a family in your area. Participation in carpool is completely voluntary. (Note: The carpool database is crowd-sourced, voluntary, and not officially managed or verified by OPUSD or the school or PFA.)
- OPEF Donation Form: The Oak Park Education Foundation raises money from parents, individuals, and local businesses, and makes direct contributions to the District annually to benefit children enrolled in all levels of Oak Park schools. All donations are tax-deductible and help maintain the high quality of education for our children!
MCMS PFA Information
The Parent Faculty Association (PFA) is a volunteer group of parents and faculty who work together organizing events and providing financial funding for activities that best benefit the students and faculty of Medea Creek Middle School (MCMS). Money raised through PFA membership and our annual Give to Grow fundraiser goes directly to enriching the educational experience of all students. For more information, please visit our website.
