Vendor Approval Process

To be compliant and become an approved vendor with OPUSD, please provide the following documents and email them to Hannie Low at hlow@opusd.org. The process to become an approved vendor may take 4-8 weeks. We may ask for more documents during this process, so please kindly plan accordingly. If you have any questions, please email Hannie Low at hlow@opusd.org.

ALL VENDORS must complete, sign and submit the following documents (document #1 and #2, click on the links below for information):
(Example: Event Vendors - DJ, Face Painters)

1. OPUSD Service Agreement
2. Certificate of Insurance with endorsements - OPUSD COI Sample (typically 4 pages)
   • (Please note that some rides/inflatables require higher insurance)

Additional documents needed if you are:

FOOD VENDORS (Example: Shaved Ice Truck)

3. Ventura County Environmental Health Permit
4. Ventura County Seal of Inspection
5. Food Safety Certificate / Food Handler Card

ANIMALS VENDORS (Example: Therapy Dogs/ Animals Exhibit)

3. Veterinary Vaccination/ Health Records and Handler Permit (Therapy Dogs)
4. California State Department Permit or UCDA Certificate (Animals Exhibit)
